The Queensland Government has strict requirements regarding providers possessing all medical information for Kindergarten students enrolled in their service. For your child to be allowed to commence in Kindergarten, the College requires all relevant medical information, including Medical Action plans and reports and/or letters from specialists, no later than Friday, 8 December 2023. Approved Action Plan Templates are available below and must adhere to the following:
Management plans must be:
- A scanned colour copy (photos of plans cannot be accepted)
- Have all sections completed by your child’s doctor
- Contain a current photo of your child (Allergy, Anaphylaxis and Coeliac only)
- Have no section of the plan missing or not visible
Reports and letters from Doctors and Specialists must be:
- Signed and dated by the Doctor/Specialist
- Contains the child’s full name
- Contains contact information for the Doctor/Specialist.
